• 3 doses of Dtap/DTP/DT or td. (Tetanus vaccines.)
• 3 - 4 doses of Polio vaccine. (Depending on age when administered.)
• 2 doses of Measles vaccine.
• 1 dose of Mumps vaccine.
• 1 dose of Rubella vaccine.
• 3 doses of Hepatitis B vaccine.

If your child receives additional immunizations during his/her high school years, it is recommended to send the documentation to the school nurse to include in the student's immunization record. This insures that the record will be complete if needed in the future for college applications of employment applications.
