Dear Parents,

 

The Indiana State Department of Health has notified all schools of new immunization requirements for the coming school year 2010-11.  All students will be required to have the following immunizations by the beginning of the school year.

1.      All kindergarten students will be required to have 2 doses of varicella (chickenpox) vaccine, given on or after the first birthday and separated by 3 month, or a physician written documentation of history of chickenpox disease, including month and year of disease.

2.      All kindergarten students will be required to have one of the required doses of polio vaccine given on or after the fourth birthday, and at least 6 months after the previous dose.

3.      All 1st grade students are required to have 1 dose of varicella (chickenpox) vaccine on or after the first birthday or physician written documentation of history of chickenpox disease, including month and year of disease.

4.      ALL 6th – 12th grade students will be required to have 2 doses of varicella (chickenpox) vaccine, given on or after the first birthday, and separated by age-appropriate intervals as defined by the CDC, or a history of chickenpox disease.

5.      All 6th – 12th grade students will be required to have 1 dose of tetanus-diphtheria-acellular pertussis vaccine (Tdap) given on or after the 10th birthday.

6.      All 6th – 12th grade students will be required to have 1 dose of meningococcal conjugate vaccine (MCV4).

Please review your child’s immunization history with your physician to ensure their immunizations are in compliance with Indiana law.  All immunizations must be current by the start of the 2010 school year. 

Free immunizations are available every Monday, except holidays, through the Delaware County Shots for Tots program at the Gateway Clinic which is located in the Muncie YWCA building at 310 East Charles Street.  Clinic hours are 9:30-11:30am and 1-3:30pm.  On the 2nd Monday they remain open until 6pm.  A record of ALL previous immunizations MUST accompany the child.  If any adult other than the parent or guardian brings the child for shots, they must have WRITTEN PERMISSION FROM THE PARENT OR GUARDIAN.  The note should include name of parent, child and adult accompanying the child as well as the clinic date.  You may contact the Health Department with questions at 747-7814. 

 

The Hepatitis A vaccine may be required in the future.  Please ask your health care provider about obtaining this vaccine for your child also.
